What is the purpose of this specific heat exchanger? Why is this needed?

The exhaust heat exchanger is used to maintain the temperature of the hybrid battery pack.
I don't believe that the exhaust heat exchanger is even part of the coolant loop for the Hybrid equipment.
Unless I read the service manual wrong, it is part of the traditional ICE cooling loop.

Below is the coolant loop(s) for the Hybrid equipment onboard. It's really rather amazing.

I think Bruce is right! The "Exhaust Heat Exchanger" (item 20) is indeed connected to the "Heater Core" (item 21) which is responsible for cabin heating.

Thank you everyone for posting so many details!! Helped me to diagnose the same problem on my brand new 2021 F150 Platinum. Dealer is not able to look at the truck for another 3 weeks, and that is with me not needing a loaner. When I made the appointment the tech was very aware of this issue and told me to keep putting coolant in if I needed to keep driving it.

So far I have put in over 2 gallons of coolant and just ticked over 1,300 miles. Truck was built in Aug of 2021 and purchased in March.

Hope for everyone the parts turn up soon, I am not looking forward to being without my truck for a long period of time.
