I have rough idle (500rpm) on occasion though mostly warm, and I have shuddering/surging around 1000RPM in high gears at low load (40mph, 9th-ish gear). I think I read that 3.5s have had shaky idles, something like low oil pressure; was true on earlier models.

I can't imagine; I stomped down on my Livernois-tuned 3.5EB in the rain at 45mph and if it wasn't for that low level stability control that you can't turn off I probably would have totaled the truck. It gets bent out of shape so fast. Have you done any suspension mods to help with control?
